The problem
Clinicians completing NHS-format forms spent significant time translating unstructured clinical notes and source documents into standardised form fields. The manual process introduced compliance errors and created backlogs in document-heavy workflows.
What we built
An AI system that generates completed NHS medical forms from two inputs:
- Unstructured clinical prompts — the clinician’s notes, dictated or typed
- OCR of source documents — existing patient records, referral letters, test results
The system extracts relevant clinical data, maps it to the correct form fields, flags missing required information, and outputs a completed form ready for clinician review and sign-off.
